The VA's Spin

Oy. I don't know why perfectly good companies, individuals, and institutions feel the need to take impressive records and harm their credibility by spinning them into extraordinary stories, but in the VA's case, it'll only hurt them. McClatchy News does seem to have the goods on the system trumping up some of their stats, particularly how rapidly they grant appointments. But before folks get too excited, the VA's lead in care delivery, chronic disease management, and systems integration has been examined over and over again, and RAND, The New England Journal of Medicine, The Annals of Internal Medicine, and The National Committee for Quality Assurance have all ranked them first. It would be better if they weren't exaggerating their gains, but such misdeeds don't harm the empirical case.

All that said, McClatchy is certainly right that the VA faces serious difficulties in dealing with returning Iraq veterans, both their physical losses -- new medical technology means the grievously injured live to be rehabilitated -- and their mental traumas. The VA system, like many other critical government departments, isn't being given near the resources to deal with the coming influx, and the consequences will be disastrous. This isn't a structural problem in the VA, but in our government: It's our duty to give veterans the care they need, and that requires money. We have the institutions and delivery mechanisms in place, they just need to be properly funded.

Sacrifice! An income tax surcharge of the progressive type seems like a funding source for the VA that it would be very hard for the Bushies to oppose.

Since those with money favored the Bush/Cheney admin which brought the nation the horrors of damaged brains and whacked out mental health in Iraq with their voting and contribution support, and these folks have sacrificed nothing - let them pay a large part of the long term cost of treating our vets as they deserve.

Adding to my comment above: the surcharge should be set to expire in, say, 50 years, so everyone gets the message that these costs of health care are generational each time the war hawks get us in deep doo-doo (as Bush I would be prone to say).

Of course if the government would just do this and that... This is fundamentally THE reason that a market-oriented solution will achieve your means, not a single-payer system.

Single payer, where governments makes economically rational, efficient decisions are great in theory. In practice, however, that just isn't what happens. Government makes decisions that are inherently political, and the resulting system deviates from what is theoretically best for patients. This is the whole reason that capitalism outperforms socialism in the long run.

Let me point out an exmaple that went under the radar, and ask that you actually look into the details here. Right around Christmas, when everyone else was busy with their families, Congress pushed through another classic midnight-hour piece of Medicare legislation-- which dealt with issues of physician compensation, pay-for-performance incentives, etc. No transparency, no discussion-- in the end, this stakeholder group got what they wanted, because they lobbied the right people. This was the most significant legislation passed with respect to CMS in the past year. This isn't new, it happens every year.

Change in our government-funded health care system is driven by politics, special interests, lobbying, etc. That is fundamentally the problem with a single-payer system-- the stakeholders will shape the system, not the "health care wonkosphere."

Your comment is silly and simplistic. Espousing a market approach is not a defense of today's system. You are falsely implying that we have a black-and-white choice: stick with the current system or move to single payer.

One can address universal access and other issues without moving to a single-payer system.

Decisions that are inherently political are not always better then deicisions that are inherently profit based, i.e. market based.

In practice governments all over the world make health care choices that are more popular, cheaper and in many instance produce better outcomes then choices made here by private companies.

Hm. Dramatically increasing VA funding under the Democratic auspices would be an incredibly wise long-term political strategem. (It would, coincidentally, also be the right thing to do.) Getting Iraq veterans to look kindly upon Democrats will do a lot to prevent the historical revisionism that happened in the wake of Vietnam. The Iraq veterans will be the nation's moral authority on war for the next fifty years; it'd be good to get them on our side now.
